Elder Law
   
  Health Care Proxies
   
  A Health Care Proxy allows you to appoint an agent to make medical decisions on your behalf.
  Your Proxy is only effective during periods when you are unable to make a reasoned decision for yourself.
  A valid Health Care Proxy helps avoid a Guardianship for medical matters.
   
  Living Wills
   
  A Living Will indicates your wishes with regard to discontinuance of life support, or other “heroic measures”.
  Without a Living Will or other strong evidence of your beliefs regarding continued life support, you likely will be kept alive by artificial means.
  A Living Will does not mean that in the event of an emergency you will not receive emergency treatment or be resuscitated.
   
Powers of Attorney
   
  A Power of Attorney allows your appointed agent to engage in financial and insurance transactions.
  The document may be designed for a single transaction or allow your agent to do anything you personally would be able to do.
   
Agent for Disposition of Remains
   
  Without this document, State law will determine which of your next-of-kin have control of your remains and funeral arrangements.

  For Incapacitated adults.
  For mentally retarded/developmentally disabled persons.
